What wisdom teeth are, and why they cause trouble

Wisdom teeth are the last molars at the back of the mouth, usually arriving in the late teens or twenties. There is often not enough room for them, so they come in at an angle, stay buried in the jaw (impacted), or only partly break through the gum. A partly erupted tooth traps food and bacteria under the gum flap, which leads to repeated swelling and pain.

Signs a wisdom tooth may need removing

Not every wisdom tooth has to come out. A tooth that has come through straight, can be cleaned and causes no problems can usually stay. The X-ray decides, and you are told plainly when removal is not needed.

Simple or surgical removal

A wisdom tooth that has fully come through can often be removed like any other tooth. An impacted or partly erupted one needs a surgical extraction: a small opening in the gum, sometimes removing a little bone or dividing the tooth into pieces so it comes out gently. That is the work of an oral surgeon, and it is done at the clinic.

What happens, step by step

  1. Examination and X-ray. The position of the tooth, its roots and the nerve nearby are checked before anything is planned.
  2. Your plan. You are told whether the tooth needs to come out, whether it is a simple or surgical removal, and what recovery to expect.
  3. The removal. Done under local anaesthetic, so you feel pressure but not pain. Stitches are placed if needed.
  4. Follow-up. A check a few days later, and stitches removed if they are not the dissolving kind.

Recovery and aftercare

Some swelling and stiffness for two to four days is normal, especially after a surgical removal. Use a cold compress on the cheek for the first day, eat soft food, avoid smoking and drinking through a straw, and do not rinse hard for the first 24 hours so the blood clot stays in place. Gentle warm salt-water rinses help after that. Call the clinic if pain gets worse after the third day, or if swelling spreads.

Frequently asked questions

Does wisdom tooth removal hurt?

It is done under local anaesthetic, so you feel pressure rather than pain. Soreness and swelling afterwards are normal for a few days and settle with the painkillers you are prescribed.

Do all wisdom teeth need to be removed?

No. A wisdom tooth that has come through straight and can be cleaned usually stays. Removal is advised when it causes pain, infection or damage to the tooth in front.

How long does recovery take?

Most people are back to normal routine within a few days. Swelling peaks around the second or third day after a surgical removal and then settles.
